Do Fat-Dissolving Injections Make You Lose Weight? What Lemon Bottle Does and Does Not Do

Dr. Sinan Akyürek explains how Lemon Bottle fat-dissolving injections work, where they make sense, how many sessions, who should skip it, why it is not a diet.

Dr. Sinan Akyürek Clinic — targeted local fat and realistic expectations

There is a question I hear often in consultations: “Doctor, if I have three sessions of this fat-dissolving injection, how many kilos will I lose?” The question itself is innocent. The real issue is who taught people to ask it. On social media this injection is presented as a kind of liquid diet. People imagine lying at home while the doctor comes over, gives an injection right here, and poof, all the fat is gone. Unfortunately, that is not how it works.

My answer is zero kilos. That does not mean the treatment is bad. It means it gives the right answer to the right question. First I will explain what it is, and then we can talk about which question it answers.

Is this a weight-loss method?

No. General weight loss does not come from an injection; for that you need diet and exercise. When you lose weight, the fat stores throughout the body shrink at the same time. Lemon Bottle goes to a single area, targets the fat there and leaves the rest of the body alone. That is why expecting the number on the scale to change is the wrong expectation.

So why does that area need a separate treatment? Because some fat deposits are stubborn. You lose weight, your face slims down, your waistline shrinks. But that small fullness under the chin, or that soft band across the lower belly, stays where it is. The body gives up the fat in certain areas last, and sometimes never. This is exactly the spot my patients mean when they say “I’m already slim, but this part won’t go,” or “I’m losing weight, but I want the belly and the sides to go first.”

So we mostly use this treatment in patients whose weight is where it should be but who cannot make peace with one or two areas. I say this in the first minute of the examination, because if you don’t set the expectation there, disappointment after three sessions is inevitable.

What is Lemon Bottle, and what is in it?

Lemon Bottle is a lipolysis solution injected with a fine needle into the fat layer under the skin. It belongs to the family people call fat-dissolving injections, which we call injection lipolysis in our own language. It contains riboflavin, which is vitamin B2, bromelain, an enzyme obtained from pineapple, and lecithin.

How does it work? Fat is stored inside the fat cell, and as long as it stays there, the area looks full. The solution aims to get that fat out of the cell. The fat that comes out is carried away by your own body; the lymphatic system and the liver do this job. In other words, the injection releases the fat and your metabolism takes over the transport. That is why I ask for plenty of water, regular movement and light eating between sessions. If the released fat is not carried away, it waits where it is.

In the past we did local slimming with mesotherapy. It took five or six sessions spread over four or five months; if people did not start in January or February, they could not be ready by summer. Lemon Bottle has shortened this time noticeably for us.

Where does it make sense?

In limited, well-defined areas. Fullness under the chin comes first; many of my patients have a double chin that runs in the family, regardless of weight. The lower belly below the navel, the inner upper arm, the inner thigh and the bulges above the bra strap are other areas I see often. What they have in common is this: an area about the size of a palm, a soft layer of fat you can hold between your fingers, and skin above it that has kept its elasticity.

Not every fullness is fat, either. Especially under the chin. Not everyone has a mass of fat there. There are two lymph glands in that area, the windpipe and the gullet pass between them, and the base of the tongue and the hyoid bone sit there too. These structures already have a volume of their own. If the jawbone is small or set a little back, they start shouting “I’m here.” In a patient like this, bringing the chin forward a little with filler gives a better result. If the tight bands we call platysmal bands have become prominent with age, sometimes a simple botox treatment is enough. And sometimes what sags is loosened skin or a muscle that has lost its tone.

This is why, during the examination, I hold the fat layer between my fingers and check its thickness. If there is no fat to hold, I don’t do this injection; that day the patient may be directed towards skin quality or tightening instead. If some laxity remains in an area where we have reduced the fat, we talk about tightening it with a treatment that increases collagen production.

For a patient with widespread excess fat over large areas, I say it openly: unfortunately this injection will not reduce that volume. That belongs to weight management or to surgery. Sometimes I need to send this patient to a different door.

What Lemon Bottle does and does not do: the difference between targeted local fat and weight loss

How many sessions, and when do results show?

One session does not finish the job. We usually plan two to three sessions, two to four weeks apart. The reason is that carrying away the released fat takes time. With each session the area gets a little smaller, and at the next session we look at what is left and carry on from there.

In most of my patients the first difference shows after the second session. How many centimetres of slimming there will be depends on the treated area and how much fat is there; where there is a lot of fat, the slimming is naturally greater. We make the final assessment about a month after the planned sessions are over, by comparing measurements. Don’t look for the result of this treatment in the mirror the next morning. What you will see the next morning is swelling.

Lemon Bottle timeline: treatment day, swelling in the first days, softening over two to four weeks and assessment a month after the sessions

Swelling and recovery: what happens in the first week?

This is the period patients worry about most. For three to five days after the treatment the area is swollen and tender, there is a stinging feeling, and you can feel a firmness with your hand. Bruising may occur. None of this means something has gone wrong; it is exactly the reaction we expect.

Swelling is at its peak in the first two days, then comes down slowly. During this period I ask you to stay away from hot environments, saunas and heavy exercise. The firmness resolves within two to four weeks, and as the area softens, the slimming starts to show. If the treatment is going on a visible area such as under the chin, don’t schedule it right before an important invitation or meeting; give yourself a week’s margin.

Rarely, a firmness lasts longer or the surface feels uneven. We follow this up at the check-up, and most of it settles on its own. I can’t call this a painless, trace-free treatment. But it is a short and manageable process.

Who is it not suitable for?

We do not perform it during pregnancy or breastfeeding. If there is an active infection or an open wound in the treatment area, we wait. In people with a bleeding disorder, uncontrolled diabetes, or thyroid, liver or kidney disease, this treatment is not on the table. Because the bromelain in it comes from pineapple, I specifically ask about pineapple allergy; this has to be discussed in the examination.

There is also a non-medical kind of unsuitability, and that is the wrong expectation. Treating a patient who comes in saying “I’ll lose weight with this injection without dieting” would mean failing to give them accurate information. On the day, we reduce the fat in the area. But if you keep eating and drinking and keep adding on top of the work we have done, what can local slimming do? When weight is gained, the remaining cells can grow again. The broken-down fat leaves, but keeping the area that way is your homework.

The “it melted in a week” videos on social media

Some of these videos may show the result of a genuine local treatment, while others are lighting, angle and editing. You can’t tell which is which from a screen, and neither can I. The faster and more effortless a treatment is made to sound, the more questions I suggest you ask. No injection lipolysis settles in a week, because the body that carries the fat away does not work on that calendar. Even the firmness takes two to four weeks to resolve.
